Job Description :
Our client, a major Health System, is seeking RN Case Managers in Dallas/El Paso, TX area!!
Highly preferred RN that is fluent in Spanish, Mandarin, or Vietnamese
Shift: 9a-6p
RN Case Manager Job Responsibilities:
Case Manager Job Responsibilities:
Develops and implements a case management plan in collaboration with the member, caregiver, physician and/or other appropriate healthcare professionals and member's support network to address the member needs and goals. Provides daily review and evaluation of members that require hospitalization and/or procedures providing prior authorizations and/or concurrent review. Conducts face-to-face home visits as required. 25%-40% local travel required.
Required Qualifications :
Case Manager Job Requirements:
- Bachelor's degree in Nursing or Masters degree in Social Work.
- 2-4 years of clinical experience with case management
- Active, unrestricted RN TX license, LCSW, or APSW in good standing.
- Knowledge of ICD-9, CPT coding and HPC
- Must have valid driver's license with good driving record and be able to drive locally.
