RN Healthcare Transitions Coach

We have an immediate opening for an RN Health Transitions Coach to join our team in the northern Chicagoland Area! **PLEASE NOTE - candidates must reside in northern Chicago or northern suburbs region to be considered. This is a 6-month contract-to-hire position. Candidates MUST have an Illinois State RN license or LVN/LPN with clinical case management or public health course concentration, 1-2 years current or recent clinical case management or public health experience to qualify - no exceptions!

Hours: full-time; 40 hrs/wk

Salary: up to $36/hr (based on experience)

Essential Responsibilities:
Responsible for safely and effectively transitioning and discharging members from acute or inpatient care to lower levels of care and/or home in a cost efficient manner. Provides assessment, planning, implementation, coordination, monitoring, and evaluation of services for Molina members as they transition care and follows them for 30 days post discharge. 

• Completes clinical assessments of members per regulated timelines and determines who may qualify for case management based on clinical judgment, changes in member's health or psychosocial wellness, and triggers from the assessment.
• Develops and implements a case management plan in collaboration with the member, caregiver, physician and/or other appropriate healthcare professionals and member's support network to address the member needs and goals.
• Conducts telephonic, face-to-face or home visits as required.

• 1-2 years of current or recent clinical case management or public health experience

Required Education:

• Bachelors degree (preferred) or LVN/LPN with clinical case management or public health course concentration from an accredited college or university

Required License(s):

• Valid, unrestricted IL State RN license in good standing highly desired, but not required
